Introduction

The death certification process in England and Wales is about to have long overdue and significant change, the importance of which has been highlighted in numerous reports and enquiries including:

  • The Shipman Report
  • The Report of the Mid Staffordshire NHS Foundation Trust Public Inquiry
  • The Morecambe Bay Investigation Report
  • The government response to the report of the Gosport Independent Panel

The date for introduction is 9 September 2024.

The existing system of medical certification by a medical practitioner or investigation by a coroner will be supplemented by a new system of scrutiny by a body of independent medical examiners.

New categories of informants who can register a death have also been added.
